New Patient Registration Form
Complete before your first visit. Covers personal information, emergency contacts, and basic dental history.
Medical & Dental History Form
Helps us provide safe, personalized care. Includes current medications, allergies, and relevant health conditions.
Consent to Treatment Form
Standard informed consent required before dental procedures begin. Reviews your rights and treatment options.
NIHB Verification Form
For First Nations and Inuit patients using Non-Insured Health Benefits. We submit your NIHB claims directly.
Insurance Assignment Form
Authorizes Edmonton West Dental to bill your insurance provider directly, so you only pay your share at checkout.
Privacy Policy Acknowledgement
Required for all new patients. Confirms how we collect, use, and protect your personal health information.
